Insurance Required for Tenants

**Requirements as written in Lease

• Worker's Compensation:
  Statutory Amount (checkmark in WC Statutory Limits box on certificate)
• Employer's Liability:
  $1,000,000 minimum
• Commercial General Liability:
  $5,000,000 Combined Single Limit for Bodily Injury and property damages
• Commercial Automobile Liability:
  $1,000,000 each occurrence combined single limit for bodily injury and property damage

Certificate of Insurance Requirements - Contractor/Vendor

All Contractors and Vendors conducting any business at the 55 West Monroe Building must have an original Certificate of Insurance on file with the Management Office that meets the following requirements:

1. Comprehensive General Liability insurance against claims for property damage, personal injury, bodily injury and death occurring upon, in or about the premises. Liability insurance may be carried under one or more umbrella policies, aggregating the minimum combined single limit described and shall (a) be on the occurrence form with a combined single limit of not less than $5,000,000 and (b) cover the following hazards: (i) Premises and operations (ii) Products and completed operations on an "if any basis" (iii) Independent contractors (iv) Blanket contractual liability (v) Contractual liability
2. Commercial Automobile Liabity insurance covering owned, non-owned and hired automobiles, trucks and trailers used by the Contractor in the minimum amount of $1,000,000 combined single limit for Bodily Injury and Property Damage.
3. Property insurance coverage for tools and equipment brought onto and/or used on any Property by the contractor – an amount equal to the replacement costs of all such tools and equipment.
4. Worker's compensation, subject to the statutory limits of the state in which the services are to be rendered. Employers' liability insurance with a minimum of $1,000,000 per accident per employee and $1,000,000 per disease per employee. Also $1,000,000 disease aggregate with respect to any work or operations upon, in or about the building and in connection with the building.
5. The certificate of insurance must contain a thirty (30) days cancellation notice mailed to the Certificate Holder in the event of policy cancellation or change.
6. Contractor's Commercial General Liability, Automobile Liability and Umbrella Liability insurance policies shall be primary and non-contributed. Please include verbiage as such in the description section.
7. The insurance coverage shall be issued by an insurance company acceptable to building management, owners and lenders and have a general policy rating of (A) or better and a financial class of (VI) or better by A.M. Best Company, Inc.
8. Service Contractor waives any and all rights of subrogation against the parties identified below as additional insureds. Please mark both boxes for Additional Insureds and Subrogation Waiver on the COI accordingly for all insurance policies.
